What is an Education, Health and Social Care (EHC) plan?

Education, Health and Care (EHC) Assessment and Review Team 

 

The EHC Assessment and Review Team is responsible for carrying out EHC needs assessments for children and young people with high needs aged 0 to 25 years. 

 

They liaise closely with parents, education setting and a range of services to ensure the provision made for those with Education, Health and Care Plans (EHCP) is both co-ordinated and matched to their need.

Meet the team 

A local authority education and healthcare team is a group of people who work together to help kids who need extra help with their learning or health. They make plans to make sure these kids get the right support at school and with their health needs. This team includes teachers, doctors, and other experts who care about helping these kids do well and feel good.
