Keyworker Project

Early Support Keyworker Team at Rochdale 

 

The Early Support Keyworker Team is a home visiting service for babies and young children who have additional needs or disabilities. We are based within the SEN and Children with Disabilities Team located in Number One Riverside, Rochdale.

 

What we do 

All our team members are trained and qualified to work with children with additional needs and have a wide range of experience supporting children and their families.

We carry out joint visits to the children with other therapists in the home.  We also work alongside parent/carers within the home to support children's development.  We provide learning through play activities and experiences that build on children's strengths.

 

Referral criteria and information 

For children aged 0 to 3 years old with complex health needs or a disability and living in Heywood, Middleton and Rochdale. Children do not need to have a diagnosis to access the service but must have significant development delay in more than one area.

We expect that the child will have at least one term with us before starting nursery or school and that they are not already in a setting.

Children will have regular reviews to ensure that the service continues to meet their needs and remains appropriate. Anyone can refer a child to us. We will then make a decision taking into account the child’s individual needs and our referral criteria.

 

We except open referrals to service and we accept referrals from both parents and professionals.
