NHS Community Health in Heywood, Middleton and Rochdale

We have two main health care providers in Heywood, Rochdale and Middleton for children's Health. 

Northern care alliance - NCA 

 

Children’s Community Integrated Health Services (used to be CAON)

 

The service is for children and young people aged from birth up to 19 years who live or are registered with a GP in Heywood, Middleton or Rochdale (HMR). 

 

The service provides advice, information, assessment, intervention, care, treatment and review services to children and young people and their families. Care and treatment is delivered in a range of settings across Heywood, Middleton and Rochdale, including people's own homes, nurseries, children's centres, special schools and clinics.

-       Children's Community Nurses

-       Children’s Epilepsy Specialist

-       Children’s Diabetes Specialist

-       Children’s Occupational Therapy

-       Children’s Respiratory Specialist

Welcome to Children’s Community Integrated Health Services

The service provides advice, information, assessment, intervention, care, treatment and review services to children and young people and their families. Care and treatment is delivered in a range of settings across Heywood, Middleton and Rochdale, including people's own homes, nurseries, children's centres, special schools and clinics.

The service is multi-disciplinary, which means lots of different professionals work together as one team. This allows the service to provide flexible and joined up care and children, young people and their parents and carers will only have to tell their 'story' once, rather than to several different professionals, leading to better care and a better experience.
